The Milwaukee County Department of Family Care operates the Family Care Managed Care Organization (MCO) through a contract with the State of Wisconsin Department of Health Services (DHS). We provide managed long-term care services to frail elders (age 60 and older) and people age 18–59 who have physical, intellectual and/or developmental disabilities.
We believe that if you need long-term care services, you should be able to get them when you need them and at a place you choose. Our goal is to work with you to design a plan to provide you with cost-effective care that meets your outcomes.
Since 2000, we have provided the Family Care program to over 21,720 people. Today, we serve over 8,300 members. For more information on how this program began, click here.
All Family Care members must qualify financially and functionally for the program. For more information about enrollment and eligibility, click here.
